Rifabutin


Pronunciation: RIF-a-BUE-tin
Generic Name: Rifabutin
Brand Name: Mycobutin


Rifabutin is used for:

Preventing Mycobacterium avium complex (MAC) disease in patients with advanced HIV infection.


Rifabutin is an antibacterial. It works by preventing or slowing the growth of certain bacteria.


Do NOT use Rifabutin if:


  • you are allergic to any ingredient in Rifabutin or to any other rifamycins (eg, rifampin)

  • you have active tuberculosis (TB) or kidney problems

  • you are taking an HIV protease inhibitor (eg, ritonavir) or voriconazole

Contact your doctor or health care provider right away if any of these apply to you.



Before using Rifabutin:


Some medical conditions may interact with Rifabutin. Tell your doctor or pharmacist if you have any medical conditions, especially if any of the following apply to you:


  • if you are pregnant, planning to become pregnant, or are breast-feeding

  • if you are taking any prescription or nonprescription medicine, herbal preparation, or dietary supplement

  • if you have allergies to medicines, foods, or other substances

  • if you have kidney problems

Some MEDICINES MAY INTERACT with Rifabutin. Tell your health care provider if you are taking any other medicines, especially any of the following:


  • Many prescription and nonprescription medicines used for infections (eg, voriconazole), blood thinners, high cholesterol, birth control, heart problems, HIV infection (eg, ritonavir), mental or mood problems, seizures, or diabetes, and herbs found in multivitamin products, herbal teas, and dietary supplements (eg, garlic, St. John's wort) may interact with Rifabutin and increase the risk of side effects

This may not be a complete list of all interactions that may occur. Ask your health care provider if Rifabutin may interact with other medicines that you take. Check with your health care provider before you start, stop, or change the dose of any medicine.


How to use Rifabutin:


Use Rifabutin as directed by your doctor. Check the label on the medicine for exact dosing instructions.


  • Take Rifabutin by mouth with or without food. If stomach upset occurs, take with food to reduce stomach irritation.

  • Do not wear contact lenses while you are taking Rifabutin. Sterilize contact lenses according to manufacturer's directions and check with your doctor before using them.

  • To clear up your infection completely, take Rifabutin for the full course of treatment. Keep taking it even if you feel better in a few days.

  • If you miss a dose of Rifabutin, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not take 2 doses at once.

Ask your health care provider any questions you may have about how to use Rifabutin.



Important safety information:


  • Rifabutin may discolor the urine, stools, saliva, sweat, tears, and skin (usually a brown-orange color). This is normal and not a cause for concern. It may also permanently stain soft contact lenses. If possible, do not wear contact lenses while you are taking Rifabutin.

  • Rifabutin only works against bacteria; it does not treat viral infections (eg, the common cold).

  • Be sure to use Rifabutin for the full course of treatment. If you do not, the medicine may not clear up your infection completely. The bacteria could also become less sensitive to this or other medicines. This could make the infection harder to treat in the future.

  • Long-term or repeated use of Rifabutin may cause a second infection. Tell your doctor if signs of a second infection occur. Your medicine may need to be changed to treat this.

  • Rifabutin may lower the ability of your body to fight infection. Avoid contact with people who have colds or infections. Tell your doctor if you notice signs of infection like fever, sore throat, rash, or chills.

  • Mild diarrhea is common with antibiotic use. However, a more serious form of diarrhea (pseudomembranous colitis) may rarely occur. This may develop while you use the antibiotic or within several months after you stop using it. Contact your doctor right away if stomach pain or cramps, severe diarrhea, or bloody stools occur. Do not treat diarrhea without first checking with your doctor.

  • Hormonal birth control (eg, birth control pills) may not work as well while you are using Rifabutin. To prevent pregnancy, use an extra form of birth control (eg, condoms).

  • Lab tests, including complete blood cell counts, may be performed while you use Rifabutin. These tests may be used to monitor your condition or check for side effects. Be sure to keep all doctor and lab appointments.

  • Rifabutin should not be used in CHILDREN; safety and effectiveness in children have not been confirmed.

  • PREGNANCY and BREAST-FEEDING: It is not known if Rifabutin can cause harm to the fetus. If you become pregnant while taking Rifabutin, contact your doctor. You will need to discuss the benefits and risks of using Rifabutin while you are pregnant. It is not known if Rifabutin is found in breast milk. Do not breast-feed while taking Rifabutin.


Possible side effects of Rifabutin:


All medicines may cause side effects, but many people have no, or minor side effects. Check with your doctor if any of these most COMMON side effects persist or become bothersome:



Changes in taste; diarrhea; discolored urine; headache; indigestion; loss of appetite; nausea; sleeplessness; stomach pain; vomiting.



Seek medical attention right away if any of these SEVERE side effects occur:

Severe allergic reactions (rash; hives; itching; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); bloody stools; chest pain; eye pain, swelling, or sensitivity to light; flu-like symptoms (eg, fever, chills, sore throat); severe or persistent diarrhea; severe stomach pain or cramps; unusual bruising or bleeding; unusual muscle pain or weakness; vision changes or decreased vision; yellowing of skin or eyes.

If OVERDOSE is suspected:


Contact 1-800-222-1222 (the American Association of Poison Control Centers), your local poison control center, or emergency room immediately.


Proper storage of Rifabutin:

Store Rifabutin at 77 degrees F (25 degrees C). Brief storage at temperatures between 59 and 86 degrees F (15 and 30 degrees C) is permitted. Store away from heat, moisture, and light. Do not store in the bathroom. Keep Rifabutin out of the reach of children and away from pets.

What is Reclipsen (ethinyl estradiol and desogestrel)?

Ethinyl estradiol and desogestrel contains a combination of female hormones that prevent ovulation (the release of an egg from an ovary). This medication also causes changes in your cervical mucus and uterine lining, making it harder for sperm to reach the uterus and harder for a fertilized egg to attach to the uterus.


Ethinyl estradiol and desogestrel are used as contraception to prevent pregnancy.


Ethinyl estradiol and desogestrel may also be used for other purposes not listed in this medication guide.


What is the most important information I should know about Reclipsen (ethinyl estradiol and desogestrel)?


Do not use birth control pills if you are pregnant or if you have recently had a baby. Do not use this medication if you have any of the following conditions: a history of stroke or blood clot, circulation problems (especially if caused by diabetes), a hormone-related cancer such as breast or uterine cancer, abnormal vaginal bleeding, liver disease or liver cancer, severe high blood pressure, migraine headaches, a heart valve disorder, or a history of jaundice caused by birth control pills.

You may need to use back-up birth control, such as condoms or a spermicide, when you first start using this medication. Follow your doctor's instructions.


Taking hormones can increase your risk of blood clots, stroke, or heart attack, especially if you smoke and are older than 35.

Some drugs can make birth control pills less effective, which may result in pregnancy. Tell your doctor about all the prescription and over-the-counter medications you use, including vitamins, minerals and herbal products. Do not start using a new medication without telling your doctor.


What should I discuss with my healthcare provider before taking Reclipsen (ethinyl estradiol and desogestrel)?


This medication can cause birth defects. Do not use if you are pregnant. Tell your doctor right away if you become pregnant, or if you miss two menstrual periods in a row. If you have recently had a baby, wait at least 4 weeks before taking birth control pills (6 weeks if you are breast-feeding). Do not use this medication if you have:

  • a history of a stroke or blood clot;




  • circulation problems (especially if caused by diabetes);




  • a hormone-related cancer such as breast or uterine cancer;




  • abnormal vaginal bleeding;




  • liver disease or liver cancer;




  • severe high blood pressure;




  • severe migraine headaches;




  • a heart valve disorder; or




  • a history of jaundice caused by birth control pills.



Before using this medication, tell your doctor if you have any of the following conditions. You may need a dosage adjustment or special tests to safely take birth control pills.



  • high blood pressure, heart disease, congestive heart failure, angina (chest pain), or a history of heart attack;




  • high cholesterol or if you are overweight;




  • a history of depression;




  • gallbladder disease;




  • diabetes;




  • seizures or epilepsy;




  • a history of irregular menstrual cycles; or




  • a history of fibrocystic breast disease, lumps, nodules, or an abnormal mammogram.




The hormones in birth control pills can pass into breast milk and may harm a nursing baby. This medication may also slow breast milk production. Do not use if you are breast-feeding a baby.

How should I take Reclipsen (ethinyl estradiol and desogestrel)?


Take this medication exactly as it was prescribed for you. Do not take larger amounts, or take it for longer than recommended by your doctor. You will take your first pill on the first day of your period or on the first Sunday after your period begins (follow your doctor's instructions).


You may need to use back-up birth control, such as condoms or a spermicide, when you first start using this medication. Follow your doctor's instructions.


The 28-day birth control pack contains seven "reminder" pills to keep you on your regular cycle. Your period will usually begin while you are using these reminder pills.


You may have breakthrough bleeding, especially during the first 3 months. Tell your doctor if this bleeding continues or is very heavy.

Take one pill every day, no more than 24 hours apart. When the pills run out, start a new pack the following day. You may get pregnant if you do not use this medication regularly. Get your prescription refilled before you run out of pills completely.


If you need to have any type of medical tests or surgery, or if you will be on bed rest, you may need to stop using this medication for a short time. Any doctor or surgeon who treats you should know that you are using birth control pills.


Your doctor will need to see you on a regular basis while you are using this medication. Do not miss any appointments.


Store this medication at room temperature away from moisture and heat.

What happens if I miss a dose?


Missing a pill increases your risk of becoming pregnant. Follow the directions on the patient information sheet provided with your medicine. If you do not have an information sheet, call your doctor for instructions if you miss a dose.


If you miss one "active" pill, take two pills on the day that you remember. Then take one pill per day for the rest of the pack.


If you miss two "active" pills in a row in week one or two, take two pills per day for two days in a row. Then take one pill per day for the rest of the pack. Use back-up birth control for at least 7 days following the missed pills.


If you miss two "active" pills in a row in week 3, or if you miss three pills in a row during any of the first 3 weeks, throw out the rest of the pack and start a new one the same day if you are a Day 1 starter. If you are a Sunday starter, keep taking a pill every day until Sunday. On Sunday, throw out the rest of the pack and start a new one that day.


If you miss three "active" tablets in a row during any of the first 3 weeks, throw out the rest of the pack and start a new pack on the same day if you are a Day 1 starter. If you are a Sunday starter, keep taking a pill every day until Sunday. On Sunday, throw out the rest of the pack and start a new one that day.


If you miss two or more pills, you may not have a period during the month. If you miss a period for two months in a row, call your doctor because you might be pregnant.

If you miss any reminder pills, throw them away and keep taking one pill per day until the pack is empty. You do not need back-up birth control if you miss a reminder pill.


What happens if I overdose?


Seek emergency medical attention if you think you have used too much of this medicine. Overdose symptoms may include nausea, vomiting, and vaginal bleeding.


What should I avoid while taking Reclipsen (ethinyl estradiol and desogestrel)?


Do not smoke while using this medication, especially if you are older than 35. Smoking can increase your risk of blood clots, stroke, or heart attack caused by birth control pills.

This medication will not protect you from sexually transmitted diseases--including HIV and AIDS. Using a condom is the only way to protect yourself from these diseases.


Reclipsen (ethinyl estradiol and desogestrel) side effects


Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficulty breathing; swelling of your face, lips, tongue, or throat. Stop using this medication and call your doctor at once if you have any of these serious side effects:

  • sudden numbness or weakness, especially on one side of the body;




  • sudden headache, confusion, problems with vision, speech, or balance;




  • chest pain or heavy feeling, pain spreading to the arm or shoulder, nausea, sweating, general ill feeling;




  • a change in the pattern or severity of migraine headaches;




  • nausea, stomach pain, low fever, loss of appetite, dark urine, clay-colored stools, jaundice (yellowing of the skin or eyes);




  • swelling in your hands, ankles, or feet;




  • a breast lump; or




  • symptoms of depression (sleep problems, weakness, mood changes).



Less serious side effects may include:



  • mild nausea, vomiting, bloating, stomach cramps;




  • breast pain, tenderness, or swelling;




  • freckles or darkening of facial skin;




  • increased hair growth, loss of scalp hair;




  • changes in weight or appetite;




  • problems with contact lenses;




  • vaginal itching or discharge;




  • changes in your menstrual periods, decreased sex drive; or




  • headache, nervousness, dizziness, tired feeling.



This is not a complete list of side effects and others may occur. Tell your doctor about any unusual or bothersome side effect. You may report side effects to FDA at 1-800-FDA-1088.


What other drugs will affect Reclipsen (ethinyl estradiol and desogestrel)?


Some drugs can make birth control pills less effective, which may result in pregnancy. Before using this medication, tell your doctor if you are using any of the following drugs:



  • acetaminophen (Tylenol) or ascorbic acid (vitamin C);




  • an antibiotic;




  • phenylbutazone (Azolid, Butazolidin);




  • St. John's wort;




  • seizure medicines such as phenytoin (Dilantin), carbamazepine (Tegretol), topiramate (Topamax), and others;




  • a barbiturate such as amobarbital (Amytal), butabarbital (Butisol), mephobarbital (Mebaral), secobarbital (Seconal), or phenobarbital (Luminal, Solfoton); or




  • HIV medicines such as amprenavir (Agenerase), atazanavir (Reyataz), indinavir (Crixivan), saquinavir (Invirase), fosamprenavir (Lexiva), ritonavir (Norvir), and others.



This list is not complete and there may be other drugs that can affect birth control pills. Tell your doctor about all the prescription and over-the-counter medications you use. This includes vitamins, minerals, herbal products, and drugs prescribed by other doctors. Do not start using a new medication without telling your doctor.

Dilaudid-HP


Pronunciation: HYE-droe-MOR-fone
Generic Name: Hydromorphone High-Potency
Brand Name: Dilaudid-HP


Dilaudid-HP is used for:

Treating moderate to severe pain in narcotic-tolerant patients who require larger than usual doses to provide adequate pain relief.


Dilaudid-HP is an opioid (narcotic) analgesic. It works by binding to certain receptors in the brain and nervous system to reduce pain.


Do NOT use Dilaudid-HP if:


  • you are allergic to any ingredient in Dilaudid-HP or any other codeine- or morphine-related medicine (eg, morphine, codeine, oxycodone)

  • you are NOT already receiving large amounts of injectable narcotics

  • you have difficult or slowed breathing, chronic obstructive pulmonary disease (COPD), or right-sided heart problems (cor pulmonale), or you are having a severe asthma attack

  • you are in labor

  • you have severe diarrhea or other bowel problems caused by antibiotics or poisoning

  • you have a brain injury or growths in the brain along with increased pressure in the brain

  • you are taking sodium oxybate (GHB)

Contact your doctor or health care provider right away if any of these apply to you.



Before using Dilaudid-HP:


Some medical conditions may interact with Dilaudid-HP. Tell your health care provider if you have any medical conditions, especially if any of the following apply to you:


  • if you are pregnant, planning to become pregnant, or are breast-feeding

  • if you are taking any prescription or nonprescription medicine, herbal preparation, or dietary supplement

  • if you have allergies to medicines, foods, or other substances

  • if you are allergic to latex

  • if you have a history of lung or breathing problems (eg, asthma, emphysema, bronchitis), seizures (eg, epilepsy), adrenal gland problems, an enlarged prostate, heart problems, low blood pressure, dehydration, low blood volume, an underactive thyroid, or urinary blockage

  • if you have severe drowsiness, a recent head injury, spinal problems, growths in the brain, or increased pressure in the brain

  • if you have liver or kidney problems, gallbladder problems, or stomach or bowel problems (eg, inflammation), or if you have had recent stomach or bowel surgery

  • if you drink alcohol regularly, have symptoms of alcohol withdrawal, or have a history of suicidal thoughts or attempts

  • if you have a personal or family history of mental or mood problems, alcohol abuse, or other substance abuse or dependence

  • if you are in poor health or shock, or will be having surgery

Some MEDICINES MAY INTERACT with Dilaudid-HP. Tell your health care provider if you are taking any other medicines, especially any of the following:


  • Phenothiazines (eg, chlorpromazine) because the risk of low blood pressure may be increased

  • Barbiturate anesthetics (eg, thiopental), cimetidine, or sodium oxybate (GHB) because the risk of severe drowsiness, coma, or slowed or difficult breathing may be increased

  • Agonist/antagonist analgesics (eg, pentazocine) or naltrexone because they may decrease Dilaudid-HP's effectiveness and withdrawal may occur

This may not be a complete list of all interactions that may occur. Ask your health care provider if Dilaudid-HP may interact with other medicines that you take. Check with your health care provider before you start, stop, or change the dose of any medicine.


How to use Dilaudid-HP:


Use Dilaudid-HP as directed by your doctor. Check the label on the medicine for exact dosing instructions.


  • Dilaudid-HP is usually given as an injection at your doctor's office, hospital, or clinic. If you will be using Dilaudid-HP at home, a health care provider will teach you how to use it. Be sure you understand how to use Dilaudid-HP. Follow the procedures you are taught when you use a dose. Contact your health care provider if you have any questions.

  • Do not use Dilaudid-HP if it is cloudy or discolored, or if the vial is cracked or damaged.

  • Keep this product, as well as syringes and needles, out of the reach of children and pets. Do not reuse needles, syringes, or other materials. Ask your health care provider how to dispose of these materials after use. Follow all local rules for disposal.

  • If you are taking Dilaudid-HP for persistent pain, take it on a regular schedule to help control the pain more effectively.

  • Do not change your dose or suddenly stop taking Dilaudid-HP without checking with your doctor.

  • If Dilaudid-HP is no longer needed, dispose of it as soon as possible. Ask your doctor or pharmacist how to dispose of Dilaudid-HP properly.

  • If you miss a dose of Dilaudid-HP and you are taking it regularly, take it as soon as possible. If several hours have passed or if it is nearing time for the next dose, do not double the dose to catch up, unless advised by your health care provider. Do not take 2 doses at once.

Ask your health care provider any questions you may have about how to use Dilaudid-HP.



Important safety information:


  • Dilaudid-HP may cause drowsiness, dizziness, blurred vision, or lightheadedness. These effects may be worse if you take it with alcohol, other opiate pain medicines, or certain other medicines. Use Dilaudid-HP with caution. Do not drive or perform other possibly unsafe tasks until you know how you react to it.

  • Do not drink alcohol or use medicines that may cause drowsiness (eg, sleep aids, muscle relaxers) while you take Dilaudid-HP; it may add to their effects. Ask your pharmacist if you have questions about which medicines may cause drowsiness.

  • Dilaudid-HP may cause dizziness, lightheadedness, or fainting; alcohol, hot weather, exercise, or fever may increase these effects. To prevent them, sit up or stand slowly, especially in the morning. Sit or lie down at the first sign of any of these effects.

  • This product contains a very large amount of hydromorphone. Do not confuse this product with other injectable forms of hydromorphone. If you have any questions about whether you should use this product or another injectable form of hydromorphone, contact your doctor or other health care provider.

  • Dilaudid-HP may be habit-forming. Do NOT take more than the recommended dose or use for longer than prescribed without checking with your doctor. Misuse or abuse of Dilaudid-HP may cause severe side effects, including severe breathing problems, seizures, coma, and possibly death.

  • Constipation is a common side effect of Dilaudid-HP. Talk to your doctor about using laxatives or stool softeners to prevent or treat constipation while you use Dilaudid-HP.

  • If your pain continues or becomes worse or if you have side effects that concern you, contact your doctor.

  • Tell your doctor or dentist that you take Dilaudid-HP before you receive any medical or dental care, emergency care, or surgery.

  • Use Dilaudid-HP with caution in the ELDERLY; they may be more sensitive to its effects, especially drowsiness and breathing problems.

  • Dilaudid-HP should be used with extreme caution in CHILDREN; safety and effectiveness in children have not been confirmed.

  • PREGNANCY and BREAST-FEEDING: Dilaudid-HP may cause harm to the fetus. If you think you may be pregnant, contact your doctor. You will need to discuss the benefits and risks of using Dilaudid-HP while you are pregnant. Dilaudid-HP is found in breast milk. Do not breast-feed while taking Dilaudid-HP.

When used for long periods of time or at high doses, Dilaudid-HP may not work as well and may require higher doses to obtain the same effect as when originally taken. This is known as TOLERANCE. Talk with your doctor if Dilaudid-HP stops working well. Do not take more than prescribed.


Some people who use Dilaudid-HP for a long time may develop a need to continue taking it. People who take high doses are also at risk. This is known as DEPENDENCE or addiction. Dependence is unlikely to be an issue in terminally ill patients where comfort is more important. If you are taking Dilaudid-HP regularly, do not suddenly stop taking it without checking with your doctor. WITHDRAWAL symptoms have occurred when Dilaudid-HP is suddenly stopped and may include anxiety; diarrhea; fever, runny nose, or sneezing; goose bumps and abnormal skin sensations; nausea and vomiting; pain; rigid muscles; seeing, hearing, or feeling things that are not there; shivering or tremors; sweating; and trouble sleeping. Contact your doctor if you notice any of these symptoms after stopping Dilaudid-HP.



Possible side effects of Dilaudid-HP:


All medicines may cause side effects, but many people have no, or minor, side effects. Check with your doctor if any of these most COMMON side effects persist or become bothersome:



Constipation; dizziness, drowsiness; dry mouth; flushing; lightheadedness; nausea; sweating; vomiting.



Seek medical attention right away if any of these SEVERE side effects occur:

Severe allergic reactions (rash; hives; itching; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); fainting; fast, slow, or irregular heartbeat; hallucinations; mental or mood changes; seizure; severe or persistent dizziness or drowsiness; severe or persistent headache or vomiting; shallow, slowed, or difficult breathing; tremor; trouble urinating; vision changes.

If OVERDOSE is suspected:


Contact 1-800-222-1222 (the American Association of Poison Control Centers), your local poison control center, or emergency room immediately. Symptoms may include bluish skin; chest pain; cold and clammy skin; coma; difficult or slow breathing; excessive sweating; limp muscles; nausea and vomiting; numbness of an arm or leg; pinpoint pupils; seizures; severe drowsiness or dizziness; slow or irregular heartbeat.


Proper storage of Dilaudid-HP:

Store Dilaudid-HP at 77 degrees F (25 degrees C). Brief storage at temperatures between 59 and 86 degrees F (15 and 30 degrees C) is permitted. Do not store Dilaudid-HP in the bathroom. Keep Dilaudid-HP out of the reach of children and away from pets.
